I am the payer on the accounts I have indicated for cancellation above, and understand if I am not, the membership(s) will not be cancelled, and the payer will need to submit this form.

I understand in order to cancel, any outstanding fees must be paid.

I understand the membership(s) will remain active until the cancellation is confirmed via email, which can take up to 72 hours. My account will be charged during this time.

I understand the membership(s) with an outstanding balance will be cancelled, but I remain responsible to pay all outstanding balances associated with the membership(s). Failure to pay outstanding balances may result in the account being referred to a collections agency, and legal action may be taken to recover the amounts.
